Completion of Patient Forms

At Magnolia Health, we want to support our patients by completing forms that are needed for school, work, disability, or other personal needs. To ensure accuracy and fairness, we follow the timelines below for processing requests:

Standard Timeline

  • Routine Forms (work, school, clearance, FMLA, etc.)
    Please allow 7 business days for completion after all necessary information and signatures have been received.
  • Complex Forms (disability, detailed medical records, insurance appeals, etc.)
    Please allow 10 business days, as these may require chart review and provider input.

Expedited Requests

  • If a form is needed sooner than the standard timeline, please let us know at the time of submission.
  • Expedited requests may be accommodated within 3 business days when possible, but may include a fee.

Requirements for Processing

  • Forms must be submitted in full, with patient information completed prior to submission.
  • Incomplete forms will not be processed until missing sections are completed.
  • The provider may require an appointment to complete forms if additional information or an updated evaluation is needed.

Notification

  • Patients will be contacted when forms are ready for pickup, or they may be uploaded to the patient portal when appropriate.
