1Scheduling the Meeting
Building Administrators will verify the total amount of OSS and ISS (if not receiving services) days in Eschool. If 10 days of OSS and ISS (if not receiving services) are accumulated then an MDR must occur immediately. A student is “not receiving services” during ISS if the student is not afforded the opportunity to continue to appropriately participate in the general curriculum and continue to receive the services specified on the student's IEP.
If a student is suspended from the bus (transportation) which they are eligible for as a related service within their IEP or Section 504 Plan, and they do not attend school because alternative transportation is not made available to the student, the suspension from the bus is considered a day of OSS.
Building Administrators will contact their Special Education Supervisor to determine if they will LEA the MDR meeting.
Building Administrators will consult with their Special Education Supervisor to obtain information whether the student is being recommended for expulsion.
Case manager will check that the required IEP MDR Participants are available for the meeting:
Parent
Student (if appropriate)
Psychologist
Social Worker
Gen Ed Teacher
Related Service Staff
LEA
Building Administrator (if not the LEA)
Case Manager
Bilingual/HI interpreter if needed (complete required request forms)
Case manager will contact the parent and ensure availability while providing the required 10 days prior written notice or inquire about waiving the 10 day notice.
Ask the guardian how they would prefer to receive their written notice of conference: either to be picked up, emailed, or sent via US mail.
Record parent contact within Embrace and on the conference summary report page of the IEP.
Case manager will complete the Notice of Conference checking the following boxes

Consider the needs for a Functional Behavior Assessment for your child and a need to create or revise a Behavioral Intervention Plan.
Case manager sends Notice of Conference to parent/guardian along with the Procedural Safeguards.

2IEP Team Preparation
Box 1: INCIDENT(S) THAT RESULTED IN DISCIPLINARY ACTION
A description of the most recent Eschool Behavior Incident and amount of days out.
All previous Eschool Incident(s) that resulted in out of school suspension.
Include summary of reasons for suspension, date it occurred, and amount of days out.
Box 2: THE STUDENT'S IEP AND PLACEMENT
Student’s Eligibility
Brief description of student characteristics that resulted in eligibility determination.
Current placement and services they receive (include specific minutes).
Behavior Intervention Plan (if student has one include description of target behavior).
Box 3: OBSERVATIONS OF THE STUDENT
General Education, Special Education teacher, and Related Service provider (if applicable) summary of typical observed student behavior.
Note if new student behavior was observed on the day of the incident.
Student’s shared relevant information (when appropriate).
Statement of analysis of behaviors observed across the learning environments.

Box 4: INFORMATION PROVIDED BY THE PARENTS

Potential Guiding Questions:
Are there medication changes?
Are there updated health concerns?
Are there any significant life changes?
Discuss and determine if the conduct was caused by or had a direct and substantial relationship to the student’s disability.
- *It may be helpful for the IEP team to discuss how the student is eligible for special education, restating the characteristics and eligibility criteria.
Discuss and determine if the conduct was the direct result of the school district’s failure to implement the IEP.
Check the appropriate box if the student’s behavior WAS NOT or WAS a manifestation of her/his disability.
If the behavior WAS NOT a manifestation, discuss disciplinary procedures as applicable.
If the behavior WAS a manifestation, the team must review and/or revise the IEP as appropriate including adjusting the Behavior Intervention Plan (if student has one) or obtaining consent to begin a Functional Behavior Assessment.
Any adjustments in the IEP, must be amended
In the Additional Notes capture the details of the conduct, observations shared, any other discussion, and final determinations.

4After the Meeting
Provide finalized copy of MDR, Additional Notes, and Amendment of the IEP (if completed) to parent
Send copy of finalized paperwork to Central Office: Special Education Department
