IEP Amendments (Changes to the IEP between Annual Review Dates)

1Overview
The IEP documents should tell a story about the student and outline the efforts and outcomes the team
has made working with the student. It is understood that each IEP must be reviewed no less than once a
year. However, student needs can change throughout the school year and require the team to make
adjustments in an effort to meet the needs of the students.
Below you will find an updated guidance on making minor changes to an active IEP through an
amendment. This is a consistent special education department guidance across all D100 schools. Each
amendment needs to be completed in Embrace with explicit parent contact and change documented.
The IEP Amendment is used when making minor changes to an active IEP. This does not change the
annual review date. It also does not change student programming or serving school as those are
considered major changes.

2Amendments with a Meeting
The purpose of the IEP Amendment with a meeting is to
discuss what Amendment changes need to take place that are minor. For example, changing a
class/schedule, but not changing the programming or serving school of a student can be considered a
minor change. In this circumstance a student’s Present Levels, Goals, and Accommodations would
remain the same. Another example would be to make minor changes to the BIP, such as adding another
positive reinforcement. Proper *Notice of Conference must be sent within 10 days given, check Other
and add Discuss Amendment.

3Amendment without a Meeting
The purpose of the IEP Amendment without a meeting is to make
minor changes to the active IEP during the year the IEP is in effect. If substantial changes need to be
made, the student’s IEP team must convene in person or virtually.

4Reminder of Requirements
The IEP amendment does not reset the due date of the next annual review.
The IEP amendment must be attached to the IEP that is being modified.
The parent must receive a copy of the previous IEP with the amendment attached.
The full IEP team must be notified of the changes to the IEP.
