    THIS SOP WILL BE UNDER-DEVELOPMENT FOR THE FALL 2025 AS WE CHANGE PROCEDURES FOR THE FALL25/WINTER26.

    School District Code for Forms (activity summary and evidence of completion):  040041000 - 26 
    1. 1

      PD Activity Summary

      Instructor identifies the details outlining the PD and completes the Professional Activity Summary

      Summary is sent to the designated administrator for review with accompanying documents (agenda, session presentations materials, etc).

      These materials must be sent at least 2 weeks prior to the scheduled date of the event.

      Admin Review (CO or Building Level)

      Designated administrator does the following:
      • Reviews the Professional Activity Summary and provides feedback as needed to finalize for approval
      • Sends approved PD Activity Summary to the HR Department - PD Coordinator

      HR Department Review

      Designated HR staff member - PD Coordinator does the following:
      • Using the Professional Activity Summary and provides feedback as needed to finalize for approval
        • Creates an online/Google folder for the PD event within their department specified folder
        • Creates event within the KickUp Learning Management application
        • Event is then shared either as a link to the event allowed to be viewed/browsed by all employees within the Kickup Events.
        • Evaluation (Feedback) and Evidence of Completion (Certificate) will be made available after participate registers for event and event is completed.

      State Reporting

      District Level OP completes the following form for state reporting purposes (outside of district providers only).

      This form is due by June 30 each year.

      HR PD Coordinator will submit forms yearly to the state.

      Evaluation (Feedback)

      The evaluation (called Feedback in KickUp) is located in the event they register for. Participants must go into Kickup to access the evaluation or the presenter provides a QR code to complete attendance and the evaluation (feedback). 

      Participants can access their Evidence of Completion (Certificate) at anytime in KickUp. The 5 school days is now removed.

      PDHs

      PDHs will be auto generated and available in the KickUp application for the attended event. 

      Participants should print the form for their record and input the information into ISBE's website for their PD hours gained. 

      Professional development credit is awarded on a 1:1 basis – 1 clock hour of attendance = 1 professional development hours.

      Closing out the PD Experience

      Designated Admin/Presenter:
      • Ensures that all documents related to the training are housed in the digital folder for review by the District Level OP
      • Emails District Level OP to close out the training session
      • If the training was paid, please make sure attendance sheet is provided. 
      District Level OP:
      • Ensures that applicable matrices listed above (state and local) are completed.
