Risk Assessment (Harm to Self)
If a student is at risk for harming themselves or has attempted to harm themselves, a risk assessment would be completed.

1Risk Assessment- Initial StepDetermine if the student is an immediate threat to themselves.
First, if there is an imminent danger (attempting to severely harm themselves in the moment) address that safety concern before initiating this process. (Call law enforcement, notify building administration and follow district safety guidelines.)
Then, notify CO administration starting with Chief Academic and SEL Officer or designee. -
2Risk Assessment- Harm to SelfInterview should be completed with a school district mental health/student services staff member and/or a school administrator.

Utilize the Level of Risk Document when making a determination of severity.
Place a copy in the student's temporary file and provide a copy to admistration. -
3Other Considerations/ FormsDepending on the situation/level of risk, the following forms may be utilized:
- Parent Release Form- when releasing at-risk student to the parent
- Release of Information Form- between school and Dr., agencies, etc.
- Safety Plan- in most cases this would be moderate or high risk

4HospitalizationIf a student is hospitalized either through the school's risk assessment process, on their own accord, or through the support of their family/friends, etc., school personnel should follow this process:

This includes holding a re-entry meeting and developing a safety plan (or reviewing/modifying a current plan). -
5Documentation- School/District LevelEach school should add information to their designated spreadsheet for risk assessments and/or hospitalizations.
