    Include all necessary information

    • Tooth number(s)
    • Shade
    • Material selection
      • Include a note if you would like us to choose.
      • Call if you would like help selecting the best material.
    • Notes
    • Requested return date
  2. 2

    Include all necessary scans

    • Preparation 
    • Opposing
    • Bite
    • Pre-preparation if needed to copy for fit to partial or night guard. Also useful to confirm occlusion. Check for holes and inconsistencies before submitting.

    Include any additional information

    • On multiple unit cases, specify if the units requested should be
      • single units
      • splinted together
    • If an implant is requested, include
      • system
      • connection type
      • platform size
    • Photos (for shade match, midline, lip line, occlusal plane, etc.) For shade match:
      • make sure the incisal edge of the tooth lines up with the incisal edge of the shade tab.
      • take the photo where the lighting is sufficient.
      • watch out for bad glare.
