Beal Wellness' primary lab partner is Quest Diagnostics. Unless you request an alternate laboratory, we will send your specimens to Quest Diagnostics. We recommend that you create a Quest Diagnostics patient portal account to track your financial balances and lab results directly with Quest Diagnostics.

I hereby authorize the release of medical information related to the laboratory services provided to me and authorize payment directly to Quest Diagnostics. I agree to assume responsibility for payment of charges for laboratory services that are not covered by my healthcare insurance coverage.
